Compliance: New CDL Medical Exam Forms Mandatory

Compliance Reminder

As of April 20, 2016, new compliance regulations require that the new Medical Examination forms.  The Medical Examination Form is MCSA-5875 and the Medical Examiner’s Certificate is MCSA-5876. The new forms must be used for all driver medical exams.

All previous forms can no longer be used. As a result, if an old form is used by mistake, it could lead to disqualification of a driver’s CDL if not corrected. Be sure to remind your drivers to look at the form number in the upper left corner. If they don’t see the correct number on the form, they should question if the proper forms is being used.
